Scenario

Getting a Policy From PHLY
Getting a policy from PHLY is a straightforward process. To obtain a policy from PHLY, work with your client to complete the following steps:
- Complete, sign, and date a PHLY Non-profit and Human Services application (and any additional supplemental applications listed above.)
- Complete a standard ACORD application.
- Submit currently valued insurance company loss runs for the current policy period plus three prior years.
- Submit photographs of the location(s) to be included in the coverage.
- Submit brochures, advertising materials, and website information.

- Head Start Application: New and renewal applications are available for entities predominantly operating as a Head Start center
