Documents Required for Proposal

  • Mental Health Supplemental Application
  • Brochure and/or website address
  • 3 years plus current year, hard copy, currently valued, carrier loss runs
  • Worker’s Compensation loss runs must also be submitted on any account with residential exposure

Accident Insurance

Excess Accident Medical Expense coverage is available for Mental Health Organization accounts. This highly recommended coverage is available through a partnership between PHLY and Special Markets Insurance Consultants, Inc.
