- Currently valued insurance company loss runs for the current policy period and three years prior (Not needed for accounts under $5,000 in premium)
- Copy of participants waiver
Accident Insurance
Minimum $25,000 Accident Medical Expense coverage
required on Amateur Sports, Leagues or Martial Arts.
Available through a partnership between PHLY and Life Insurance Company of North America and CIGNA Life Insurance Company of New York.
